Quit-Connection
TelASK Quit-Connection is software specifically designed to assist in the management of smoking cessation programs in healthcare. Developed in collaboration with the Ottawa Heart Institute to support the Ottawa Model for Smoking Cessation , it has been deployed at over one hundred and thirty healthcare sites in Canada and the United States and has reached over 50,000 smokers since 2006.
FEATURES:
- Cloud-Based (Software as a Service) hosted in secure Internet data centers in United States and Canada.
- Accessible on-line by authorized persons only from authenticated hospital or clinic IP addresses. No downloads are necessary.
- Each organization’s data is housed on separate dedicated servers and all transmissions are encrypted.
- Your program manager assigns system privileges to users on an administrative dashboard and credentials are easily enabled and disabled.
- The system architecture is scalable and capable of supporting large networks of hospital inpatient and outpatient programs, as well as primary care programs across widely dispersed health networks.
- Training for users is available through regularly scheduled webinars and takes less than one hour.
ASK, ADVISE
- Supports cessation programs based on the full “5 A” model (Ask, Advise, Assess, Assist, and Arrange) or the abbreviated (Ask, Advise, Refer).
- Integrates with Electronic Medical Records Systems and supports several modes of secure data transfer such as HL7, web services or SFTP. Duplication detection feature deals with multiple enrollments of the same patient from different sites.
- Options for documenting a patient’s tobacco use, willingness to quit and confidence level include:
- A Secure Online Screening Tool where healthcare providers login securely from any desktop or mobile device. Computer aided intake interviews (optimized for desktops, tablet PCs and iPads) facilitate gathering of information from patients and offer familiar milestones to help ensure consistency of process.
- Self Enrollment by Telephone where patients self enroll by calling in to a site specific toll free telephone number to complete an IVR tobacco use survey.
- MDS (North American Quit Line Consortium minimal data set) can be collected automatically using IVR if desired.
ASSESS, ASSIST, ARRANGE
- Based on the tobacco use survey results Quit-Connection assigns the smoker one of the following Quit Profiles:
- Ready to Quit
- Recently Quit (low efficacy)
- Recently Quit (high efficacy)
- Not Ready to Quit Right Now
AUTOMATED FOLLOW-UP
- Quit-Connection conducts automated personalized follow-ups with patients for up to six months using IVR, email or smart phone App. Communications are timed to coincide with known relapse points and algorithms are designed to capture information about current smoking status, confidence level and use of quit medications and provide motivational messaging.
- Results of all interactions with participants are stored in the database and an overview of participants’ quit progress can be reviewed online in real-time. Evidence has shown that patients tend to report truthfully during IVR and/or online follow-up sessions.
CONNECTION TO CARE (TRIAGE)
- During automated follow-up participants whose responses indicate they would benefit from live counseling can be “warm transferred” to third party smoking cessation specialists within the healthcare organization or at a Quit Lines. Quit-Connection helps specialists to increase their productivity by enabling them focus their efforts on those patients who need help when they need help.
- Specialists can also be immediately notified electronically (by pager or email) when a patient requires counseling. Patients requiring counseling are also flagged on the Quit-Connection dashboard.
- Interactive assessment screens for documenting live interventions by counselors are available. These screens can be used to record information gained from and provided to smokers during live interactions.
- Quit-Connection has successfully integrated with the Canadian Cancer Society Smokers Helpline, the Province of British Columbia Quit-Now Services and the New York State Smokers Helpline.
BUSINESS INTELLIGENCE TOOLS
Quit-Connection Business Intelligence Tools accurately predict the number of live interventions that will be required by patients in the weeks and months ahead. This feature enables managers to schedule appropriate levels of human resources well ahead of time.
REPORTING MODULE
A variety of reports can be generated in real time by managers, referring healthcare providers or other authorized persons including patient demographics, site smoking prevalence, intent to treat quit rates and a time stamped audit of interactions with the smoker from initial intervention to final follow-up. These reports will demonstrate the extent to which your organization is meeting tobacco accountability measures.
